Picking up where the original Crank ended, Chev (Jason Statham) is scooped up off the street after falling from a helicopter. He wakes up as a prisoner in a makeshift hospital room with a fake heart in his chest and his real heart packed in a cooler. Chev escapes the hospital with the cooler only to find that his fake heart has a very limited battery life. Chev encounters numerous obstacles as he tries to live long enough to get his real heart back in his chest.

West finds that he has become entangled in an international conspiracy where everyone is a threat.
